ANALYSIS OF THE RISK OF LUBRICATION SYSTEM FAILURE DUE TO WATER CONTAMINATION USING FISHBONE DIAGRAM AND FMEA AT PT PQRST

This study aims to analyze the risk of lubrication system failure due to water contamination at PT PLN Nusantara Power PLTMG Arun Expansion, and to determine improvement priorities based on risk levels. Water contamination in lubricating oil is a critical factor that can reduce system performance, accelerate component wear, and increase the potential for operational failure. The methods used in this study are the Fishbone Diagram to identify the root causes of failure, and Failure Mode and Effects Analysis (FMEA) to evaluate risk levels based on the parameters of Severity (S), Occurrence (O), and Detection (D), resulting in the Risk Priority Number (RPN). The results show that environmental factors and component damage are the dominant causes contributing to failure risk. The highest RPN value is found in conditions of high humidity, which leads to condensation inside the oil tank. The recommended improvements focus on environmental control, enhancement of tank protection systems, and maintenance of critical components. The implementation of these improvements is expected to reduce the risk of failure and improve the reliability of the lubrication system.
